Στα συνταγολόγια μπορεί να έρθει για θεώρηση και ένα Κέντρο Υγείας. Άρα στην καρτέλα, το περνάμε στον τύπο συναλασσομένου: "Περιφερειακά Ιατρεία". Το πρόβλημα είναι παρακάτω (στα Βασικά Στοιχεία) που ζητά Συναλασσόμενο/Υπάλληλο και στην λίστα περιέχει όλους τους περασμένους γιατρούς. Εδώ, μπορεί να έρθει ένας υπάλληλος του Κέντρου Υγείας, του νοσοκομείου, κτλ, και να αφήσει τα βιβλιάρια προς θεώρηση. Δεν υπάρχει γιατρός στην όλη διαδικασία και δεν κρατούν στοιχεία από αυτόν που θα κάνει την δουλειά αυτή. Έτσι κι αλλιώς αυτά συμπληρώνονται στο πεδία: "Εκπρόσωπος" και "Αρ.Ταυτ. Εκπροσώπου".

Τρόπος καταχώρησης :
Συναλλασσόμενος (στοιχεία αίτησης) επιλέγεται το νοσοκομείο / κέντρο υγείας κτλ
Εκπρόσωπος/ ΑΔΤ εκπροσώπου : πληκτρολογείτε τα στοιχεία του ανθρώπου που καταθέτει την αίτηση
Βασικά στοιχεία - Συναλλασσόμενος : καταχωρείτε έναν έναν τους γιατρούς που πρόκειτε να πάρουν συνταγολόγιο. Εάν ο χρήστης δεν έχει όλες τις πληροφορίες ή θεωρεί περριττό να κρατίσει αυτήν την λεπτομέρεια, καταχωρεί τον κωδικό του νοσοκομείου. Σε αυτή την περίπτωση το συνταγολόγιο θα εμφανίζεται στο μητρώο του νοσοκομείου.
